Error adding incidents from portal

Running ver. 3.45 SVN
Adding new incidents from customer portal I get the following error:

Application Error [256]
MySQL Query Error Incorrect table name '' in /home/applicazioni/sit/includes/functions.inc.php @ line 11125

Obviously non incident is created in incidents table, but some updates are added and triggers are fired to unexistent incidents

I tried a fix substituting line 1113

$sql = "INSERT INTO `{$dbIncidents}` (title, owner, contact, priority, ";

with

$sql = "INSERT INTO `{$GLOBALS['dbIncidents']}` (title, owner, contact, priority, ";

Now I get no error adding incidents and i can see new incidents in my 'sit_incidents' db table

BUT returning to main portal page the customer sees this error (an occurrence for each new incident):

Application Warning [512]
Zero records while retrieving incident last update in /home/applicazioni/sit/includes/functions.inc.php @ line 2411


In my 'sit_updates' table of db i see new entries related to added incidents, but there is a value of 0 (zero) in fields incidentid, userid and currentowner

I can't find any related bug in Mantis. Please help, my SiT is in production ...
